As a physician-led micropractice, we intentionally care for a smaller number of patients so we can provide the kind of personalized, relationship-based care that is increasingly difficult to find in today's healthcare system. Whether you're navigating perimenopause, menopause, sexual health concerns, or gynecologic conditions, you'll work directly with a physician who takes the time to listen, educate, and partner with you in your care.
We offer both in-person and virtual visits, along with transparent pricing and flexible care options. Choose between our comprehensive membership or pay-per-visit pricing—you don't need to decide before scheduling. During your first visit, we'll discuss both options and help you choose the approach that best fits your needs, goals, and anticipated follow-up.

*We offer many laboratory tests at affordable wholesale pricing, and laboratory testing may also be billed through your insurance when preferred. Imaging and most prescription medications are typically billed through your insurance.
Six-month initial commitment. After six months, your membership continues month-to-month and may be canceled at any time.
Many patients choose membership while we're optimizing treatment, then transition to occasional pay-per-visit follow-up once they're doing well. Others prefer to remain members for the added convenience, continuity, and enhanced access.

Every consultation and follow-up includes ample time for discussion, shared decision-making, and a detailed written summary with personalized recommendations.
New Menopause & Sexual Health Consultation
90 minutes — $450
A comprehensive consultation focused on menopause, perimenopause, hormone therapy, sexual health, and other complex midlife concerns.
Breast Cancer Survivorship & Menopause Consultation
90 minutes — $450
Specialized menopause and sexual health care for patients with a personal history of breast cancer. This consultation is offered by Dr. Jordan Mendoza.
New Gynecology Consultation
60 minutes — $325
For gynecologic concerns other than menopause, perimenopause, or sexual health, including abnormal uterine bleeding, pelvic pain, PCOS, PMDD/PMS, fibroids, endometriosis, vulvar conditions, contraception, preventive gynecologic care, and routine wellness examinations.
Follow-Up Visit
30 minutes — $225
For medication adjustments, hormone optimization, laboratory or imaging review, ongoing symptom management, annual wellness visits, and established gynecologic care.

Physician Phone Visit
10 minutes — $100
Available by physician invitation for a single, very brief concern that arises between planned follow-up visits.
All pricing is subject to change without notice. Existing patients and members may be eligible for grandfathered pricing as outlined below.
An excellent option for patients seeking a one-time consultation, occasional specialty care, or maintenance follow-up once symptoms are stable.
Grandfathered Pricing
Thank you to everyone who has supported Elevated Menopause Care as the practice has grown. As a small token of appreciation, the following grandfathered pricing applies:
If your initial consultation was booked on or before July 8, 2026, you'll be grandfathered into the current pay-per-visit pricing for follow-up visits through December 31, 2026, even if your appointment takes place after the pricing change.
Patients who established care before January 1, 2026 may continue scheduling pay-per-visit follow-up appointments at $150 through December 31, 2026.
Patients who establish care between January 1, 2026 and July 31, 2026 may continue scheduling pay-per-visit follow-up appointments at $175 through December 31, 2026
Patients who are currently enrolled in the Comprehensive Membership will keep their original membership rate for as long as they maintain an active membership.

What to Expect
Your care is always individualized. Some patients feel better after just one or two visits, while others benefit from closer follow-up—especially during perimenopause or when managing more complex concerns. Once things are stable, many patients transition to follow-up just once or twice a year.
We'll decide together what follow-up schedule makes the most sense for your symptoms, goals, and preferences, so you receive the care you need—without unnecessary appointments.

In-Office Procedure Pricing
All procedure fees are separate from your visit fee and are always reviewed in advance—so there are no surprises.
Comfort During Procedures
Your comfort matters. For patients who are anxious about an office procedure, pre-procedure anxiety medication (Ativan) is available when appropriate, as long as you have someone to drive you home.
For IUD insertions and endometrial biopsies, a paracervical block (local anesthetic to numb the cervix) is also available at no additional charge. We can talk through your options beforehand and decide what will make the procedure as comfortable as possible for you.
Devices (IUDs, Nexplanon) are billed through insurance when possible.
Pathology for biopsies is billed separately by the lab.

Important Planning Note
Most procedures can be completed comfortably in the office. In rare cases, anatomy, discomfort, bleeding, or cervical stenosis may prevent a procedure from being completed safely. If this occurs, the visit and time spent attempting the procedure remain billable, but the full procedure fee will not be charged. Your safety and comfort always come first.
